Mohawk Northeast, Inc. Policy for the Protection of Private Personal Information

Applicants and employees may be required to provide to Mohawk Northeast, Inc. certain private personal information, such as a Social Security number, driver’s license number, state identification number, account number, credit or debit card number, passport number, alien registration number, health insurance identification number, or other non-public information capable of being associated with a particular individual. (“Personal Information”)

In addition, due to the nature of Mohawk’s business, the Company may collect certain Personal Information from our customers, vendors or other third parties in the course of doing business with them.

Mohawk Northeast, Inc. will maintain Personal Information collected regarding any individual confidentially. Only Mohawk employees with a legitimate business need will be provided access to an individual’s Personal Information. Mohawk strictly prohibits the disclosure of an individual’s Personal Information to a third party without the express written permission of the individual, unless permitted or required by law, or unless the information has otherwise been made public, in accordance with applicable state and federal law.

Any disclosure of Personal Information in violation of this policy may result in discipline, up to and including termination of employment, and depending on the circumstances, may subject the employee to additional penalties under state or federal law.

In addition, Mohawk Northeast, Inc. has implemented internal safeguards to protect any data, computer files, and documents containing Personal Information, and to destroy or make unreadable such information prior to disposal. In certain instances, employees may be required to comply with safeguards specific to the employee’s job duties to ensure the protection or proper disposal of Personal Information. Employees will be provided additional information regarding any safeguards applicable to them. Any employee failing to comply with applicable safeguards may be subject to discipline, up to and including termination of employment.
